Indian Stream Health Center (ISHC) is a federally qualified health center (FQHC) serving the needs of Coos County, New Hampshire and surrounding areas including Maine, Vermont, and Canada. Our 850 mile service area is one of the most remote rural regions in New England and is scarcely inhabited with 6.7 people per square mile.

Originally built more than 30 years ago, the facility has served as the medical home for residents of our service area. With a commitment to sustaining a strong primary health care system in Colebrook, that is responsive to the needs of service area residents, ISHC has grown substantially since its approval as a FQHC in January 2006.
The health center is located on a medical campus along with Upper Connecticut Valley Hospital in Colebrook, New Hampshire. ISHC wishes to be the primary care medical home for all the residents living in the surrounding communities.

Offering a variety of health services, ISHC is the place for all of you and your family’s primary care medical needs.

ISHC receives grant funding to be able to provide care for our patients despite the ability to pay.  The sliding fee scale is available to all patients that fill out a household assessment and are determined eligible based on patient's household size and income.

ISHC uses the health care team model that includes an internal medicine physician, family practitioner, physician assistants, a licensed independent clinical social worker, and nursing staff that are all genuinely committed to providing you with excellent health care.  ISHC also has a care  management team, an outreach department, and a dedicated administrative staff that bridge the gap between patient and provider.

The Health Center now provides a myriad of services which include:
  • Comprehensive primary health care with an emphasis on disease prevention and health promotion activities as well as high quality chronic disease management
  • Behavioral health care services provided by a Licensed Independent Clinical Social Worker
  • Community outreach and health promotion activities
  • Low cost prescription medication program – in 2009 ISHC subsidized $95,446 for the cost of 2,747 prescriptions to 371 uninsured patients.
  • Sliding Fee Scale program in which we reduced our fees to eligible patients based on family income – in 2010 we provided $272,499 in reduced fees to 635 uninsured or underinsured patients.  With annual support from our Federal operating grant, ISHC provided approximately $215,043 in 2010 in uncompensated care.
  • Care Management Department which provides care coordination in home visits and nursing, home  health and hospice, meals, transportation and social services.
  • ISHC works with the community to provide a range of community outreach services. The health center offers flu clinics, EDGE (Education about Diabetes in a Group Environment), application assistance, informational lectures to a variety of groups; it supports the Older, Wiser, Livelier Seniors (O.W.L.S. Program) which supports health aging activities, and many other services that allow ISHC to interact directly within the community.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
The health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
